Consent:

The team at Papillon MDC Inc., under the leadership of Dr. Mirella De Civita, will be responsible for data collection, data management, data analysis, and data reporting.

Description: Since 2020, Papillon MDC inc. has been analyzing the cost of indecision in the workplace. Thus far, we have learned that leaders can estimate the cost of a late project, a missed deadline, or the addition of more people to a project. However, what we are less sure of is whether the delay of a decision is equally costly. In this survey, you are being asked to consider the cost of indecision whether yours or that of your colleagues, managers, senior advisors, or executive members of your corporate leadership team.

Confidentiality: Your anonymity as a client and participant will be safeguarded as no name is linked to any survey result. No individual profile will be disclosed. Only consultants directly involved with this project will have access to this information, which will be in the form of prevalence data. All participants solicited for this study will be informed of any study published, with reference to the study sent directly to them. Since no names are provided, you will receive this information regardless of your decision to participant.

Advantages and Disadvantages of Participation: By participating in the initiative, you are contributing to ongoing research on the cost of indecision. There are no disadvantages to your participation aside from your time in completing the required questionnaires.

Right to Refuse Participation: Your consent is required for Dr. De Civita to use your data in any study. Please note that you can decline to provide consent without any impact on your participation. Your data will only be reviewed and not used in any study.
